Pilonidal Sinus

A pilonidal sinus is a small opening or tunnel in the cleft between the buttocks, usually near the tailbone. It differs from an anal fistula. Treatment depends on whether there is infection and whether symptoms recur; a painful swelling or discharge needs prompt medical advice.

01

Overview

A pilonidal sinus is a small tunnel or hole that forms in the skin at the top of the buttocks, near the tailbone. It can become infected and filled with pus, causing pain, swelling, and discomfort. Here’s a comprehensive overview of pilonidal sinus, including its causes, symptoms, diagnosis, treatment options, prevention strategies, and follow-up care:

02

Causes of Pilonidal Sinus

  • Hair Follicle Irritation: Pilonidal sinuses are thought to develop when hair follicles in the affected area become irritated or inflamed, leading to the formation of a cyst or abscess.
  • Risk Factors: Factors that increase the risk of developing a pilonidal sinus include prolonged sitting, excess hair growth, obesity, poor personal hygiene, and friction or trauma in the affected area.

03

Symptoms of Pilonidal Sinus

  • Pain and Discomfort: Common symptoms include pain, redness, swelling, and tenderness at the base of the spine or between the buttocks.
  • Drainage: Some individuals may experience drainage of pus or blood from the sinus tract, foul odor, and recurrent infections.

04

Diagnosis of Pilonidal Sinus

  • Physical Examination: Diagnosis is typically based on a physical examination of the affected area by a healthcare provider.
  • Imaging: In some cases, imaging studies such as ultrasound or MRI may be used to assess the extent of the sinus tract or to rule out other conditions.

05

Treatment of Pilonidal Sinus

  • Conservative Management: Mild cases may be treated with conservative measures like warm compresses, good hygiene practices, and pain management.
  • Incision and Drainage: For infected or abscessed sinuses, draining pus and debris may be necessary to relieve symptoms.
  • Surgical Excision: Recurrent or complex cases may require surgical removal (excision) of the sinus tract. Techniques like pit-picking, Bascom cleft lift, or flap procedures may be employed depending on severity.
  • EPSiT Procedure: Endoscopic Pilonidal Sinus Treatment is a minimally invasive surgical technique aimed at addressing pilonidal sinus disease while minimizing extensive open surgery.

06

Prevention of Pilonidal Sinus

  • Hygiene Practices: Keeping the area clean and dry, avoiding prolonged sitting, and maintaining good personal hygiene can help prevent development or recurrence.
  • Hair Removal: Individuals with excess hair in the affected area may consider hair removal techniques as part of preventive measures.

07

Follow-Up and Monitoring

  • After treatment, regular follow-up visits with a healthcare provider may be necessary to monitor healing, check for recurrence, and provide ongoing care and support.

In conclusion, individuals experiencing symptoms of a pilonidal sinus should seek medical attention for accurate diagnosis and appropriate treatment. Early intervention and management can help alleviate symptoms, prevent complications, and improve overall outcomes for those affected by this condition.

Your questions, answered

Does a pilonidal sinus without symptoms need an operation?

Not usually. If there are no signs of infection, a clinician may recommend observation rather than an operation. Ask what changes to look out for and seek advice if it becomes painful, swollen, bleeds or leaks pus.

What if a painful lump suddenly appears near the top of the buttocks?

A painful lump that bleeds or leaks pus can indicate infection and needs an urgent GP appointment or NHS 111 advice. An abscess may need drainage. Do not wait for a routine surgical consultation or attempt to drain it yourself.

Is draining an abscess the same as treating a recurrent sinus?

No. Drainage addresses a collection of pus during an infection. A sinus that repeatedly becomes painful or discharges may need a separate discussion about longer-term treatment. The options depend on the extent of the disease and previous treatment.

Should I shave the area to prevent another episode?

NHS advice is not to shave the affected area unless a GP advises it. Ask your clinician whether any hair management is appropriate for you, particularly around a wound. Do not assume a hair removal method is suitable simply because it is described online.

Why do wound care and recovery differ between pilonidal procedures?

Some operations leave a wound open to heal, while others close it or use a less invasive approach. This changes dressing needs and follow-up. Ask how your wound will be managed, who will provide dressings and when sitting, work and exercise can be resumed for your particular procedure.
